To register for a class, you may download the registration form from this Web site and mail it to the following address or register in person at the Rehabilitation Outreach Services office at the Grey Nuns Community Hospital. Newcomers to the programs who have not been getting regular exercise are encouraged to take the Physical Activity Readiness questionnaire (par-q) available for download below.
