Life, Accidental Death and Dismemberment and Disability Insurance

Life and Accidental Death and Dismemberment (AD&D) Insurance

Covenant Health provides 1X  the annual salary for Life and AD&D insurance to all employees in benefit eligible positions. Employees may elect to purchace Optional Life and AD&D Insurance in addition to the basic coverage, increasing the total coverage by an additional 1X.
 

Short Term Disability (STD)

STD benefits may be available if employees are absent from work due to illness or injury. This benefit begins after sick leave expires and is payable for a maximum of 24 weeks from date of disability at 60% or 66.67% of employees regular weekly salary.
 

Long Term Disability (LTD)

LTD benefits begin after 24 weeks of total disability, providing employees are unable to work. The LTD benefit amount is 66.67% of employees regular monthly salary.

Bridging Benefit (management and exempt employees)

if an employee is ill or injured and has exhaused their sick leave credits, an income replacement of 66.67% of regular weekly earnings is paid up to 24 weeks from disability. Employees must provide medical infromation to access this benefit.
